You will receive all patient-related appointment forms via a link in a text or an e-mail. To ensure a quick and seamless check-in process, please complete all of the forms in your link prior to your visit.
If this is your first appointment with us, please read this document prior to your appointment for important information regarding your visit to our practice.
Please note: If you have not been seen in our offices in THREE YEARS OR MORE, you are considered to be a “NEW PATIENT” and will be asked to fill out the “NEW PATIENT” forms.

If you need to request medical records, or a form or letter from our office:
We are committed to ensuring the privacy of our patients. All medical records are confidential and managed in accordance with HIPAA regulations. Your signature is required to release your records. All medical record requests will be completed within 30 days and have a fee in accordance with Virginia law.
We do not store x-rays, MRI films, or computer discs not done in our office. Bring them with you for your doctor to review and we will give them back to you after your visit.
If you need a form that isn’t listed, please submit your request in writing via your patient portal, stating the reason and to whom the report is to be sent.
If you request a letter, legal request, disability report, DMV forms, Metro Access forms, or FMLA forms, there is a fee paid in advance. There is an additional fee for expedited forms. Disability and FMLA forms require a questionnaire, which you complete in advance. Your doctor will make the disability determination.
